About Client Forms

Pet Palace Veterinary Clinic offers patient form(s) online so you can complete them in the convenience of your own home or office.

Download the necessary form(s), print it out and fill in the required information.

Email us your completed form(s) or bring it with you to your appointment.

New Client Form

If it is your first time visiting our clinic, we will have you fill out a new client form, found by clicking HERE.

Please print and fill out this form before you come in for your visit! If you do not have a printer, we also have copies available at our front desk.

Consent Form

If you are bringing in your pet for surgery or other in-house treatment, we will have your fill out a consent form, found by clicking HERE.

Regarding the “Procedures to be Preformed” and the “Patient’s Information” fields:

• We will fill out the “Procedures to be Performed” line upon your arrival.

• Under “Patient’s Information”, simply write your animals’ name!
